Awarded the Distinguished Conduct Medal in the London Gazette of 16 January 1919.
Cook was a native of Church, Lancashire but was living with his mother in Burnley at the outbreak of war.
The citation for the Distinguished Conduct Medal published in the London Gazette reads: - "For conspicuous gallantry and devotion to duty near Mansuriyeh on 19 October 1917. He displayed great coolness and ability in getting away the first line transport when it came under heavy fire. By his action the transport suffered no casualties."
