Awarded the Military Medal in the London Gazette of 28 September 1917. Awarded a bar to his Military Medal in the London Gazette of 10 January 1920.

Smith was a native of Burnley, Lancashire.

The Northern Daily Telegraph of 12 November 1917 reported:- BRAVE STRETCHER BEARER. Another Burnley miner to win distinction on the battlefield is Private Henry Smith, East Lancashire Regiment, who has won the Military Medal. "As a stretcher bearer he carried wounded men over open ground exposed to heavy enemy machine-gun fire." Private Smith is a single man, 28 years of age, and his home is at 27, Richard Street, Burnley. Before enlisting in 1908 he was identified with St. Mary's Catholic Church.
